NMCM - BREAKING THE CHAINS - Non-Medical Case Management Addiction and mental health support and referrals.

Who It's For: BIPOC individuals living with HIV and struggling with addiction.

What it Provides: A comprehensive counseling program that offers a safe way to recover from addiction with support services from a certified Substance Use Disorder Professional who has first-hand experience with addiction and recovery.

Mental Health Support

Goals

1. Expand Reach and Inclusivity: Increase outreach efforts to reach a broader BIPOC audience living with HIV and struggling with addiction. Increase to 19 clients per year for support and referrals.

 

2. Enhance cultural sensitivity and inclusivity in program materials and service delivery to better address the unique needs of BIPOC individuals. Enhance support groups by providing groups for Black MSM, BIPOC Trans, and African populations.

 

3. Enhance Program Effectiveness by 20%: Continuously assess and improve the counseling program's effectiveness in addressing addiction and mental health challenges.

Introduce evidence-based practices and innovative approaches to better support recovery and mental well-being.
