The THRIVE – Healthier Here Program exists to connect individuals and families in King County to the culturally relevant care, resources, and support they need to achieve their best health. We focus on addressing Health Related Social Needs (HRSNs) through trusted, community-based care coordination, empowering clients to navigate complex systems and build healthier, more resilient lives.
