Who It's For:
BIPOC individuals with a primary care doctor, who are either newly diagnosed or People Living With HIV(PLWH) and could benefit from additional support services.
What it Provides:
Medical case management to augment primary care services with access to housing, food, transportation, assistance with medications, and counseling.
Services include, but are not limited to:

Mental health & Substance Abuse Treatment

Employment support

Legal Assistance

Advocacy and negotiation

Identify barriers and promote access to health and social services

Acquire identification/social security card

Other service needs identified by client
