Medical case management for HIV

KONNECT II

Who It's For:

BIPOC individuals with a primary care doctor, who are either newly diagnosed or People Living With HIV(PLWH) and could benefit from additional support services. 

 

What it Provides:

Medical case management to augment primary care services with access to housing, food, transportation, assistance with medications, and counseling. 

Services include, but are not limited to: 

  • Health Insurance  

  • Medical, vision, and dental care 

  • Medication adherence assistance  

  • Housing Assistance 

  • Mental health & Substance Abuse Treatment  

  • Employment support 

  • Legal Assistance  

  • Advocacy and negotiation 

  • Identify barriers and promote access to health and social services  

  • Transportation Assistance  

  • Acquire identification/social security card 

  • Other service needs identified by client 

  • Support Group

  • SI Habla Espanol
