Welcome to POCAAN

We are a multicultural, multi-social service agency that addresses disparities in marginalized communities in Seattle and greater King County by offering a wealth of health services and community programs.

Three Generations

Who We Serve

African Americans represent 7% of the population in Seattle and King County, but constitute 20% of those living with HIV.

Latinx people represents 8% of the population living in Seattle and 9% of the population in King County, but constitute 14% of those living with HIV.

Since our founding in 1987, we have focused our efforts on people of color who have been historically underserved by health and community services.

Our comprehensive programs range from health and educational services, to counseling and rehabilitation for BIPOC individuals in the Seattle area.

KONNECT

Konnect offers medical case management, culturally relevant counseling, and emotional support for those who are either at risk for HIV, newly diagnosed with HIV, or not receiving care. Learn more >

This program provides a positive transition from street life to a stable living environment by providing immediate housing, food, and access to recovery including life skills and job preparedness. Learn more >

CURB helps young adults ages 18-30 who want to make positive changes in their current lifestyle towards a brighter future kick their rough life to the curb. Learn more >

CORRECTIONS CONNECTION

This comprehensive, multi-jurisdictional program helps improve reentry planning and transitional services for adults being released from prison or jail. Learn more >

BEST START HOMELESSNESS PREVENTION

This program helps decrease the risk of homelessness among youth and young adults who identify as Lesbian, Gay, Bi-Sexual, Trans-gender, Queer or Questioning (LGBTQ). Learn more >

RESTORE

RESTORE is a mentoring program that uses Restorative Justice and Behavioral Health Services to intervene in the lives of underserved youth and their families. Learn more >

TRANS* ECONOMIC EMPOWERMENT

This program assesses needs and addresses inequities faced by people of color within the transgender, non-binary/gender non-conforming, and gender diverse communities.

HIV / STI TESTING AND PREVENTION

FREE, quick, and safe HIV testing with 99.8% accurate results in under 60 seconds; post-counseling and HIV/AIDS sex education; and preventive training including PrEP and PeP. Learn more >

BREAKING ADDICTION

This program provides a safe place for people who are HIV+ and on the fence about recovery. We help expedite in-patient or out-patient treatment, provide individual care, job readiness support, and referrals to recovery housing and mental health services.

Affordable, non-urgent, full-service medical care to keep King County seniors healthy and strong. Services include general physicals and wellness care, lab testing, chronic condition management and education, acute illness treatment, and social services. Learn more >

MOCHA PREP
WELLNESS CLINIC

As the only stand-alone PrEP clinic serving the Black, Indigenous and People of Color (BIPOC) communities throughout King County, MOCHA PREP provides
pre-exposure prophylaxis (PrEP) medications to help prevent the spread of HIV/AIDS disease in people who have not yet been exposed. Learn more >
